Does any of this sound familiar?
You do not need to tick every box. One or two is reason enough to call.
- One side is weaker, and everyday movements have become a negotiation
- Words are there but will not come out, or come out wrong
- Coughing or choking has started happening at meals
- Dressing, bathing or cooking now takes help
- Memory, attention or decision-making feels different
Rebuilding function, one specific ability at a time
Inpatient rehab gets you stable. What follows is longer, slower and largely invisible to anyone outside the household — and it is where most of the recovery actually happens. Our physical, occupational and speech therapists work off one shared plan rather than three separate ones, so the gains in each area reinforce rather than compete.
Who this is for
Stroke survivors at any stage, from first weeks home through long-term rehabilitation, and the families supporting them.

What your care plan covers
Three disciplines, one plan, reviewed as you progress.
Movement and mobility
Physical therapy focused on walking, transfers and balance, practiced in your own home so what you gain is immediately usable.
Daily living skills
Occupational therapy for dressing, bathing and cooking, with adaptive techniques and equipment where they genuinely help.
Speech and swallowing
Language rehabilitation and dysphagia management, including training the family in safe feeding technique.
Preventing the next one
Blood pressure monitoring, medication adherence and risk-factor work — because a second stroke is the outcome we are all trying to avoid.
From the first call to the first visit
Four steps, no surprises, and nothing billed before you have seen the plan.
1.Call us
Tell us what has actually become difficult. No forms first, no commitment, and no obligation at the end of it.
2.We assess at home
A nurse or therapist visits and looks at how you really live — your stairs, your bathroom, the medication on your counter.
3.You see the plan
Written around your recovery, your insurance and your schedule. We verify your coverage before anything begins.
4.Care begins
On a set schedule, with the same people wherever we can, and someone reachable at any hour.

This care is delivered under Joint Commission accreditation
The same national standards your hospital is held to, applied in your home. Verify us independently under NPI 1720534662.
Common Questions
Usually not. Recovery slows but does not stop, and people regain function months and sometimes years after a stroke. It is worth an assessment before assuming the window has closed.
Only what the assessment shows you need. Some people need all three, many need one or two, and the mix changes as you improve.
That is a deliberate part of the plan. Family members are the ones present between visits, and teaching correct technique prevents injury on both sides.
Measured. Function is tracked against defined markers so improvement and plateaus are both visible, and reported to your physician.
