A swallowing problem rarely announces itself
Difficulty swallowing usually gets mistaken for fussiness or slow eating, and it is often only recognized after a chest infection. It is one of the more dangerous things we assess for and one of the most treatable. Communication difficulty is not dangerous in the same way, but it is isolating enough to be worth treating just as seriously.

Yes, though the goal shifts. With progressive conditions the work is about maintaining function and safety for as long as possible, and adapting well as things change.
Not necessarily. Texture change is one tool among several, and the aim is always the least restrictive diet that is genuinely safe.
Yes. Assessing someone eating their own food at their own table gives a far more accurate picture than a clinic ever will.
